Case Manager, Registered Nurse (RN)

Schedule: this is a PRN role that will work 8-hour shifts from 8 am to 4:30 pm. This role will fill in for our full and part-time staff and could work weekdays and weekends.

This team assists with discharge planning, ensuring patients receive the necessary resources for a smooth transition from the hospital. You'll coordinate with third-party payers, government programs, and community services to arrange appropriate care, housing, transportation, and other essential services. Your role will involve coordinating placement and support services, including long-term care, rehabilitation, behavioral/mental health, home care, transportation, and DME, as well as assisting with CPS, APS, adoptions, and end-of-life planning, including advance directives. This position offers an opportunity to make a direct impact on patient care by helping navigate complex discharge processes and fostering continuity of care.

How you'll contribute

A Case Manager, Registered Nurse (RN) who excels in this role:

  • Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
  • Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care.
  • Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.
  • Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

What we're looking for

Applicants should have a current state or compact RN license and possess an associate degree in nursing. Additional requirements include:

  • Bachelor of Science in Nursing preferred
  • Accredited Case Manager (ACM) within 2 years of hire/transfer
  • Basic Life Support (BLS) upon completion of hospital orientation
