Planned Parenthood Gulf Coast is in search of a Patient Navigator. This role supports PPGC’s goal to improve health outcomes in our service areas by assisting patients with navigating follow-up treatment and care within PPGC health centers and with linkage to care to external providers. This includes a focus on PrEP services and linkage to care for HIV prevention.

Essential Duties and Responsibilities:

  • Adheres to all PPGC, PPCFC and PPFA policies and procedures and all applicable Texas and Louisiana state laws.
  • Develop policies and procedures consistent with PPFA Medical Standards and Guidelines guidance specific to PrEP program services, develop educational/informational PrEP materials for use in clinical settings and policies and procedures to address patients concerns about ability to pay for PrEP services.
  • Facilitate provision of PrEP at home sites and assist with developing training for clinical staff to increase PrEP utilization.
  • Initiate contact with family planning providers and STD clinics to inform them of availability of PrEP services at PPGC and facilitate linkage to care with external providers as needed.
  • Prioritize other providers including LGBT service providers, Pride chapters, college campuses, SANE programs, SART teams, community action agencies, family practice providers and human service agencies for outreach.
  • Provide technical assistance to include information on, Assessment of barriers and gaps that impact the provision of PrEP, Clinical staff training, Medication adherence support, Maximizing third party reimbursement and patient assistance programs for PrEP, Enrollment in PrEP-AP, Enrollment in AIDS Institute PrEP Provider Directories.
  • Assist in ordering LARCs for eligible patients from patient assistance programs and in appropriately scheduling patients for insertion.
  • Facilitate patient navigation as needed for assigned referrals and linkage to care to external providers.
  • Coordinate with external providers for incoming referrals including but not limited to genetic counselors and testing laboratories.
  • Coordinate with organizations that provide funding and practical support to PPGC patients including but not limited to the Lilith Fund and CASN.
  • Coordinate with and participate in PPGC’s research programs, as appropriate.
  • Participate in staff training activities.
  • Understands and complies with all informed consent, mandated waiting periods and parental consent laws. Documents compliance with all laws.
  • Strictly follows all OSHA and Infection Control guidelines for reducing risk of exposure to blood borne pathogens and in the handling and disposal of hazardous waste. Uses PPE as required.
  • Understands and complies with Risk Management and Safety Procedures.
  • Complies with regulations regarding patient rights.
  • Understands and complies with Child and Minor Abuse identification and reporting.

  • Experience in providing training with adult learners
  • Experience in women’s reproductive health
  • Management, organizational and interpersonal skills
  • Ability to organize work and establish priorities, with good attention to details
  • Strong public speaking and group facilitation skills; bilingual candidates preferred
  • Must have excellent communication skills (verbal and written)

  • Able to work a flexible schedule and travel to multiple sites, as needed (including evening and weekend hours as needed)

Education and/or Experience:

  • Bachelor’s Degree in Sexuality, Education, Social Work, Public Health, related field or equivalent training and experience
  • Minimum two years providing education and/or direct service in family planning, STIs, and hard reduction.
  • Ability to assess clinical sites, communicate with clinical providers, and suggest programmatic changes
